Do you print copies of immunization records for parents to take to their children’s schools? ALERT IIS can help! The Certificate of Immunization Status (CIS) is the official form for school and child care immunization records in Oregon. You can print a child’s immunization record on a CIS form through ALERT IIS. To print this form, first find a child’s record and click on Reports.
Then choose the last report, Certificate of Immunization Status.
The immunization dates in ALERT IIS will autofill onto the CIS form and you can print this form. In addition, parents may update the form by writing the dates of additional immunizations received and signing the form.
